What you will do:

  • Undertake comprehensive Child and Family Assessments and Care Act transition assessments where applicable.

  • Complete assessments under the Children Act 1989, including Section 17 (Children in Need) and Section 47 enquiries.

  • Develop, implement, and review child-centred care and support plans to help children and young people thrive within their communities.

  • Facilitate transition planning for young people moving into adult services, ensuring continuity of care.

  • Identify safeguarding concerns, participate in child protection conferences, and manage risks in complex cases involving neglect, abuse, or family stress.

  • Work sensitively with children and young people with learning disabilities, autism, sensory impairments, and complex medical needs.

  • Support families in accessing short breaks, direct payments, and specialist services.

  • Collaborate closely with education, health, SEND services, and voluntary organisations to deliver joined-up support.

  • Prepare reports for court proceedings and contribute to Public Law Outline (PLO) processes.

  • Maintain accurate and timely case records and engage fully in supervision and ongoing training.

Person Specification:

Essential Requirements:

  • Degree in Social Work (BA / MA / DipSW)

  • Registration with Social Work England

  • Experience in children’s social care

  • Strong knowledge of disability legislation and safeguarding frameworks

  • Ability to manage complex family dynamics and work effectively in multi-agency settings

Desirable:

  • Experience within a Children with Disabilities Team

  • Experience with transition work (ages 16–25)

  • Familiarity with SEND processes and Education, Health and Care Plans (EHCPs)
