This well-bonded team is an integral part of supporting young people who are looked after by the Local Authority, providing guidance to those aged 14 and above, homeless individuals aged 16 to 18, and care leavers up to 25.

What you will do:
- Work within a stable and well-established team to support young individuals transitioning towards independence.
- Collaborate with the team manager, practitioner manager, social workers, social care officers, and young person advisors to deliver personalised care plans.
- Help engage young people in decisions affecting their lives and improve services for others through active participation.
- Establish and maintain strong relationships with partners and stakeholders, ensuring comprehensive and effective support networks for the youth.
- Perform assessments, provide guidance, and offer solutions tailored to meet the individual needs of adolescents under the care of your unit.
