An exciting opportunity has arisen for an experienced Social Worker to join the Hospital Discharge Team in Dumfries. What you will do:

  • Assessment and Discharge Planning: Complete social work assessments for adults in hospital and coordinate safe, timely discharges in line with legislation and policy.

  • Safeguarding and Risk Management: Identify and manage adult protection concerns, complete risk assessments, and contribute to protection planning while promoting independence.

  • Multi-Disciplinary Working: Work closely with NHS professionals including doctors, nurses, and therapists, attending ward rounds and discharge planning meetings.

  • Legal and Policy Frameworks: Apply relevant Scottish legislation, including the Social Work (Scotland) Act 1968 and Adults with Incapacity (Scotland) Act 2000, and contribute to Mental Health and guardianship processes where required.

  • Recording and Professional Practice: Maintain accurate and timely case records and engage fully in supervision, training, and continuous professional development.

Requirements:

  • Qualification: Degree in Social Work or equivalent recognised qualification.

  • Registration: Registered with the Scottish Social Services Council (SSSC).

  • Experience: Proven experience in adult social work or health-related settings; hospital discharge or integrated team experience is highly desirable.

  • Skills: Ability to manage complex cases, work under pressure, and collaborate effectively within multidisciplinary teams.
