Role overview

An exciting opportunity has arisen for an experienced Children’s Social Worker to join the Through Care Team in Hartlepool. This role focuses on supporting looked after young people aged 16+ and care leavers up to the age of 25, helping them navigate a successful transition into adulthood.

This is a rewarding and impactful position offering flexibility, professional challenge, and the chance to make a lasting difference in young people’s lives. The post is initially for three months, with the strong possibility of extension.

What you will do

  • Manage a caseload of looked after young people aged 16+ and care leavers up to 25.

  • Complete Pathway Plans and conduct regular reviews focusing on education, employment, accommodation, health, and emotional wellbeing.

  • Support young people to build independence, resilience, and informed decision-making skills.

  • Safeguard young people in line with statutory responsibilities, identifying and managing risks such as homelessness, exploitation, mental health needs, and substance misuse.

  • Work collaboratively with multi-agency partners to secure accommodation, education, training, and employment opportunities.

  • Build consistent, trusting relationships using strengths-based and trauma-informed approaches.

  • Advocate for young people’s rights, aspirations, and long-term outcomes.

  • Maintain accurate, timely, and high-quality case records and reports.

  • Participate in supervision, training, and service development activities to support continuous improvement.
