What you will do:

Lead and manage the Transition to Adulthood team for ages 16–25, ensuring effective service delivery and support
Develop, implement, and oversee care plans for young adults, facilitating their transition into independent living

Offer guidance and supervision to social work staff, promoting best practices and professional development
