This permanent position offers the chance to deliver high-quality, recovery-focused mental health care to adults with complex needs within a community setting.You will work autonomously while being fully supported by a skilled and multidisciplinary team, contributing to meaningful outcomes for service users through evidence-based practice, care coordination, and therapeutic engagement. What you will do Manage a caseload of adults with complex mental health needs, providing autonomous and skilled nursing care. Undertake comprehensive mental health and risk assessments, developing person-centred care plans. Implement and review care plans in line with CPA and recovery-focused models, supporting individuals in the community. Deliver evidence-based interventions, including medication management, psychoeducation, and therapeutic support. Monitor mental state and physical health, identifying and escalating concerns appropriately. Act as Care Coordinator, advocating for service users and promoting their independence and recovery. Assess and manage risks related to self-harm, suicide, and safeguarding, in line with policy and best practice. Work within the Mental Health Act and contribute to safeguarding processes. Collaborate closely with the multidisciplinary team, including psychiatrists, psychologists, social workers, and partner agencies. Support transitions from higher-intensity services to community-based care. Provide supervision and guidance to Band 5 nurses, students, and junior staff. Contribute to service development, quality improvement, and clinical governance. Maintain accurate, timely, and professional clinical documentation. Why choose this role?
