You will work as part of a supportive multidisciplinary team, delivering high-quality, person-centred occupational therapy interventions across the local area. What you will do Deliver a comprehensive community-based occupational therapy service, supporting rehabilitation and promoting independence. Complete functional assessments and develop tailored intervention and treatment plans. Support service users within their own homes and community settings to achieve meaningful outcomes. Work collaboratively with multidisciplinary colleagues to provide holistic, high-quality care. Maintain accurate, timely, and legally compliant clinical records in line with professional standards. Essential requirements Proven experience working in a community occupational therapy setting. Strong rehabilitation experience, including assessment and intervention planning. HCPC registration is essential. Access to a car and a valid driving licence to support community visits.
