Deliver strength-based interventions to enhance independence, making effective use of community resources. Complete Mental Capacity Assessments and make Best Interest Decisions where appropriate. Provide short-term intensive assessments to establish individuals’ levels of independence. Coordinate care home placements and stabilise complex cases before transfer to long-term care teams. Undertake the initial concern stage of Adults Safeguarding enquiries. Collaborate with partner organisations, including the NHS, ICB, and housing services, to deliver holistic care solutions. Essential Requirements Minimum of two years’ post-ASYE experience. Demonstrated experience managing complex caseloads within Older Persons and Physical Disability (OPPD) services. Proficiency in case management systems (e.g. Mosaic, Liquid Logic). Strong experience in safeguarding investigations. About the Team The Assessment & Intake Team provides a vital initial assessment function for Southwark’s Older Persons and Physical Disability Service.
