What You’ll Be Doing Collaborate with internal teams and external agencies to meet the needs of vulnerable adults. Place service users and carers at the heart of care planning, ensuring their voices are central to decisions. Conduct Mental Capacity Assessments and lead Best Interest decisions. Manage Court of Protection applications and attend hearings when required. Lead on safeguarding enquiries and address cases of self-neglect, ensuring legal and ethical compliance. Promote reablement, independence, and well-being by linking individuals with the right services and support pathways.
