You'll be expected to spend at least two days a week in the office, helping young people as they transition into adulthood. As part of our specialised team, you'll play a vital role in shaping the future of young adults as they move towards greater independence.

What you will do:

  • Conduct Care Act 2014 assessments and transition assessments to support young people into adulthood.

  • Create and manage person-centred transition and support plans, promoting independence and wellbeing.

  • Work with young people with learning disabilities, autism, and complex needs, ensuring smooth transitions to adult services.

  • Carry out Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.

Person Specification:

  • Degree in Social Work (BA/MA or equivalent).

  • Evidence of continued professional development.

  • Registered with Social Work England.

  • Knowledge of the Care Act 2014, Children and Families Act 2014, and Mental Capacity Act 2005.

  • Enhanced DBS clearance is required.
