You'll be expected to spend at least two days a week in the office, helping young people as they transition into adulthood. As part of our specialised team, you'll play a vital role in shaping the future of young adults as they move towards greater independence.

What you will do:

Conduct Care Act 2014 assessments and transition assessments to support young people into adulthood.
Create and manage person-centred transition and support plans, promoting independence and wellbeing.
Work with young people with learning disabilities, autism, and complex needs, ensuring smooth transitions to adult services.
Carry out Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.

Person Specification:

Degree in Social Work (BA/MA or equivalent).
Evidence of continued professional development.
Registered with Social Work England.
Knowledge of the Care Act 2014, Children and Families Act 2014, and Mental Capacity Act 2005.
Enhanced DBS clearance is required.
