What you will do:

Provide high-quality nursing care to patients in their homes during out-of-hours periods.

Work collaboratively with other members of the community nursing team to ensure continuity of care and support.

Conduct assessments, administer medications, and perform clinical procedures as required.

Offer support and advice to patients and their families, addressing any concerns they may have regarding their health.
