What you will do:
- Provide specialist dietetic assessment and treatment programmes for adults in their own homes, care homes, and intermediate care units requiring nutritional support, including Home Enteral Tube Feeding
- Make clinical judgments on nutritional status and formulate individualised treatment plans
- Recommend to GPs on the appropriate prescribing of oral nutrition supplements and enteral feeds
- Adhere to local policies and guidance on appropriate prescribing practices
- Offer training, support, and guidance on nutritional screening and prescribing practices
- Contribute to Nutritional Support and Home Enteral Tube Feeding service development, creating effective strategies, policies, and guidelines
- Undertake measurement and evaluation of your clinical work through evidence-based projects, audits, and outcome measures
- Manage your specialist caseload of at-risk clients, maintaining accurate records
Be a registered Dietitian with the Health and Care Professions Council HCPC, and hold a valid driving licence as travel between sites is necessary.
