We have an exciting ongoing opportunity for a Band 6/7 Physiotherapist within the Rehab Team in Chelsea (UK), earning up to £26-31 per hour on a full-time basis. This role is ideal for a seasoned physiotherapist, ready to consolidate core physiotherapy skills and take on complex discharge planning in a fast-paced environment. You'll be part of the Chelsea & Westminster NHS Trust, covering Acute Medical wards and a frailty ward. If you've completed core rotations at Band 7 level, this is your chance to make a significant impact while supporting junior staff.

What you will do:
- Deliver high-quality physiotherapy assessments and interventions to patients across the Acute Medical and frailty wards.
- Facilitate and execute complex discharge planning, ensuring smooth transitions for patients returning to the community.
- Supervise and mentor junior staff, fostering a learning environment that encourages skill development and professional growth.
- Collaborate with multidisciplinary teams to provide holistic care tailored to individual patient needs.
- Maintain comprehensive and accurate records of patient interactions and care plans, ensuring compliance with NHS standards.
Requirements:
- HCPC registration is mandatory.
- A valid driver's licence is required, as it may be necessary to travel between sites.
