What you will do:

Conduct CHC assessments, including Checklists, Decision Support Tools (DSTs), and Fast Track assessments.
Coordinate and participate in multidisciplinary team (MDT) meetings, ensuring robust and evidence-based eligibility decisions.
Develop and review care packages and support plans, ensuring they meet individuals’ assessed health needs.
Monitor and review existing CHC cases, ensuring continued eligibility and appropriate use of resources.

Person Specification:

Qualifications:

Registered Nurse (RMN, RGN, or RNLD).
Evidence of continued professional development.

Registration & Compliance:

Active registration with the Nursing and Midwifery Council (NMC PIN).
Strong knowledge of the National Framework for NHS Continuing Healthcare, Care Act 2014, and Mental Capacity Act 2005.
Enhanced DBS clearance.
