What you will do:
- Provide outcome-focused assessments for adults facing physical disabilities or age-related challenges, helping them reclaim their independence.
- Collaborate seamlessly with teams to ensure effective hospital discharge processes and other key services.
- Offer knowledgeable guidance and support to individuals, helping them overcome daily challenges and improve their quality of life.
- Utilise the Mosaic case management system to record and monitor progress, refining your technical skills in the process.
- Travel as needed (must have a valid driving license and vehicle) to meet clients and deliver on-site support and guidance.
Requirements of the Occupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent experience working as an Occupational Therapist in the UK
- Track record of NHS and local authority experience
- Registered with the HCPC
- Experience in hospital discharge services is essential
- Expertise in the Mosaic case management system is desirable
