What you will do:

- Conduct in-depth mental health and social care assessments, crafting person-centred care plans tailored to each individual
- Oversee and manage a caseload, ensuring that support aligns with service users' needs by regularly coordinating and reviewing care arrangements
- Perform thorough risk assessments, implementing effective risk management strategies to safeguard service users

- Collaborate actively within multi-disciplinary teams and engage with external agencies to provide comprehensive support
- Offer advocacy and assistance, championing recovery processes, independence, and social inclusion
- Handle challenging crisis situations effectively, including hospital discharges and managing high-risk cases
- Keep meticulous, timely records and produce reports to inform care decisions and service improvements
