An exciting opportunity has arisen for a Social Worker to join the SEND Family Help Team in Wolverhampton. What you will do:
Lead direct practice with families, conducting assessments and developing tailored intervention plans to support children with SEND.
Utilise your experience to carry out assessments, care planning, and crisis intervention for children with disabilities.
Implement early interventions and mobilise resources to prevent escalation of family needs.
Coordinate multi-agency support, ensuring a holistic and child-centred approach to care planning.
Apply your knowledge of safeguarding, legislation, and child protection frameworks to guide best practice.
Deliver the graduated model approach, providing personalised and evolving support based on family needs.
Support young people with SEND in preparing for adulthood, focusing on independence and life skills.
Engage in reflective supervision and contribute to team learning and continuous improvement.
