Social Worker – Transitions Team (Young Adult Disabilities)
An exciting ongoing opportunity is available for a Locum Social Worker to join the Transitions Team in Leicester, earning £34.00 per hour. This role offers the flexibility of full-time or part-time work and focuses on supporting young people with additional needs as they move from children’s services into adult social care.
You will play a vital role in assessment, planning, and coordination, ensuring positive adult outcomes, increased independence, and smooth service transitions.

What you will do:

Transitions Assessment & Planning: Complete transition assessments for young people approaching adulthood in line with the Care Act 2014, developing person-centred plans aligned with Preparing for Adulthood outcomes.

Multi-Agency Working: Work collaboratively with children’s social workers, adult social care teams, SEND services, and other partners, attending reviews and professional meetings.

Disability & Additional Needs Support: Support young people with learning disabilities, autism, and complex needs, promoting independence and community inclusion.

Mental Capacity & Legal Frameworks: Apply the Mental Capacity Act 2005 and support young people and families to understand rights and options.

Safeguarding & Risk Management: Identify safeguarding concerns, complete risk assessments, and contribute to safeguarding plans as required.

Recording & Professional Practice: Maintain accurate, timely case records, meet professional standards, and engage in ongoing training and development.
Person Specification:

Qualifications: Degree in Social Work (BA / MA / DipSW).

Registration: Registered with Social Work England (SWE).

Experience: Experience in children’s or adult social care is essential, with experience supporting young people with disabilities advantageous.
