Purpose of the role:As a Social Worker in the North Locality Team, you will work closely with adults, families, and carers to promote independence, safety, and wellbeing. You will carry out statutory duties in line with Welsh legislation, contributing to positive and sustainable outcomes within a supportive locality-based service.What you will do: Conduct strengths-based assessments of adults’ needs under the Social Services and Well-being (Wales) Act 2014. Develop, implement, and review person-centred care and support plans that promote independence and wellbeing. Complete Mental Capacity Assessments and support best-interest decision-making where required. Support adults and carers to access appropriate services, resources, and community-based support. Identify, respond to, and manage safeguarding concerns, undertaking safeguarding enquiries as necessary. Work in partnership with health professionals, housing services, and third-sector organisations to deliver holistic care. Manage a complex caseload, maintaining high-quality case records and professional reports. Support families and carers with guidance, including planning safe and timely hospital discharges. Contribute to service development and quality improvement initiatives within the locality. Person specification: Degree in Social Work. Registration with Social Care Wales. Enhanced DBS clearance.
