You will work across hospital and community settings, supporting early discharge, admission avoidance, and rapid response services, including Falls Pick-Up, Intermediate Care, and Emergency Department admission prevention. The service operates closely with social care partners, including Reablement services and hospital social workers, ensuring seamless transitions across care pathways.What you will do: Assess and treat patients using evidence-based physiotherapy interventions in acute and community settings Facilitate early hospital discharge and prevent unnecessary admissions Work collaboratively within a multidisciplinary team to deliver urgent community responses Provide specialist physiotherapy input within the UCR and Discharge Service Liaise closely with social care partners, including Reablement services and hospital social workers Contribute to rotational service cover, including Rapid Response, Falls Pick-Up, and Intermediate Care Essential requirements: HCPC registration as a Physiotherapist Previous Band 6 experience in acute and/or community settings Valid UK driving licence Strong communication and teamwork skills within multidisciplinary environments Why Whitechapel: Whitechapel offers an unbeatable mix of culture, diversity, and connectivity.
