What you will do:

• Assess, care plan, and deliver treatment to a caseload of service users and their families who are experiencing a range of mental health conditions.
• Conduct risk assessments and manage risks effectively to ensure the safety of the service user, their relatives, friends, and the wider public.
• Participate in Care Coordination and work as part of the Initial Assessment and Duty Worker rota.
• Support timely discharges from inpatient services and facilitate transfers of care, ensuring seamless continuity for service users.
• Travel within the community as needed, so being a car driver is essential for this role.
