What you will do:

- Provide clinical support to wards and inpatient areas, ensuring that patients receive the best pharmaceutical care.
- Conduct thorough medication reconciliations, clinically screen medications for both inpatient and discharge needs.
- Collaborate closely with ward teams to ensure Timely Transfer of Accountabilities (TTAs) are ready for the scheduled discharge times.
- Record clinical interventions meticulously and monitor the prescribing of high-cost drugs to ensure compliance and efficiency.
- Use your clinical expertise to make significant contributions to the pharmacy team and patient care.

Requirements:

- Registration with the Health and Care Professions Council (HCPC) is required.
- A minimum of 2 years’ experience in a similar role.
- While not essential, a full driving licence is preferable, helping you to reach various locations with ease and ensure timely service delivery.
