What you will do:

• Act as the main care coordinator, managing groups of service users through comprehensive assessments and recovery planning

• Deliver asset-based assessments, asset mapping, and personalised recovery plans using motivational interviewing techniques

• Create and coordinate tailored packages of care, including recognised psychosocial interventions, one-to-one key work, POD work, group work, and clinical interventions

• Support the development of community-based recovery groups and strengthen links with local recovery and mutual aid services

• Accurately collect, record, and report required data, ensuring timely and compliant submissions

• Assess and manage service user risk, completing risk assessments and making referrals to treatment and community support agencies

• Work closely with prescribing clinicians to enhance substitute prescribing regimes and improve social functioning

Requirements:

• Strong understanding of drug and alcohol issues and recovery-focused practice

• Experience working within substance misuse or recovery services

• Ability to work collaboratively with mental health services, criminal justice agencies, and community groups

• Excellent communication, IT, and time management skills

• Ability to work independently and manage a varied caseload effectively
