What you will do:

Collaborate closely with families and children with disabilities to ensure their needs are met efficiently and empathetically.

Conduct assessments, develop care plans, and monitor progress to provide tailored support for each child under your care.

Work with a multi-disciplinary team, maintaining strong communication and coordination to deliver cohesive support services.

Participate in case conferences and court proceedings, preparing detailed reports as required.

Handle a caseload primarily involving children in care, child protection, and children with disabilities, using your expertise to promote positive outcomes.

Coordinate with external agencies and professionals to ensure holistic support is provided to children and families.
