This role is ideal for a committed and industrious qualified Social Worker with experience in Adult Social Work, specifically working with individuals who have Learning Disabilities and Autism. We value individuals who are proactive, adaptable, and uphold high professional standards. An in-depth understanding of current legislation, including the Care Act 2014, Continuing Health Care, Mental Health Act, and Mental Capacity Act 2005, is essential, alongside knowledge of Community DOLS/LPS.
As a Social Worker in this position, you will engage with vulnerable adults and must have some understanding of physical disabilities, complex needs, and the broader community. The role requires confidence in conducting safeguarding investigations, mental capacity assessments, making best interest decisions, and executing outcome-focused assessments and support planning.
What you will do:
- Conduct assessments to determine care and support needs for adults with Learning Disabilities and Autism
- Develop and implement personalised support plans
- Collaborate with partner agencies to ensure comprehensive support
- Carry out safeguarding investigations and mental capacity assessments
- Maintain up-to-date case records and report writing
