As a Band 4 Support Worker within the Adult Disability Service at Le Grand Courtil, you will play a vital role in providing 24-hour support to adults with profound and multiple learning disabilities, empowering them to live independently in their own homes. This role focuses on delivering care through the Active Support Model, promoting independence by doing with individuals, not for them.

What you will do:

• Support individuals with personal care, medication administration, cooking, cleaning, shopping, managing finances, and accessing the community.

• Supervise and guide Band 3 Support Workers, maintaining high standards of care.

• Act as a key worker for designated service users, developing personalised support plans and maintaining accurate documentation.

• Ensure compliance with service procedures and quality standards at all times.

• Drive service vehicles (valid driving licence required) to support community access and engagement.
