What you will do:

- Deliver care to patients in their own homes or in clinic settings, ensuring high standards in all environments.
- Be responsible for a defined population of service users, providing care across various Trust locations.
- Conduct thorough assessments of care and support needs, and develop, implement, and evaluate care plans.
- Provide patient support to promote independence, improve health, and prevent unnecessary hospital admissions.
- Maintain accurate records and ensure the safe use and transport of equipment.
- Supervise and support team members as required, fostering a collaborative and effective work environment.
