What you will do:
- Conduct assessments and create bespoke care plans tailored to the individual needs of children with disabilities.
- Work directly with children, families, and other professionals to ensure a holistic and supportive care network.
- Provide advocacy and support to ensure that the voices and rights of children and families are heard and respected.
- Maintain detailed records and case notes in line with local authority requirements.
- Participate in and contribute to multi-disciplinary team meetings to offer insights and recommendations.

Requirements of the Social Worker:
- A degree or equivalent qualification in Social Work is essential.
- You must be currently registered with Social Work England.
- Demonstrable significant frontline experience in UK Social Services working as a qualified Social Worker is required.
