Work alongside experienced OTs within the Cardiology and Respiratory teams. If you are an Occupational Therapist with acute hospital discharge experience and hold HCPC registration, this is an opportunity not to miss!

What You Will Do:

Perform thorough discharge planning for patients in acute hospital settings.
Collaborate with Cardiology and Respiratory teams for comprehensive patient care.
Conduct rehabilitation tailored to individual patient needs.
Administer cognitive assessments to address patient concerns effectively.
Work closely with multidisciplinary teams to ensure seamless patient transitions from hospital to home.
Liaise with community services to facilitate ongoing patient support.
Keep accurate records and maintain high standards of patient documentation.
