What you will do:

• Conduct detailed assessments in clients’ homes, focusing on their individual needs and priorities
• Lead a pilot reablement project supporting independence and wellbeing for individuals with disabilities and long-term health conditions
• Collaborate with the Adult Helpdesk, managing referrals and delivering targeted occupational therapy interventions
• Provide expert guidance to support care needs assessments and effective care planning
• Partner with social care teams, voluntary organisations, and community services to deliver holistic support
