What you will do:

- Provide an outcome-focused assessment service for adults with physical disabilities or age-related challenges, supporting their journey towards greater independence.
- Collaborate with local authorities and hospital teams for smooth discharge processes and continuity of care.
- Utilise the Mosaic case management system for efficient and effective case management, ensuring best practices are maintained.
- Travel throughout the area as a driver with access to a personal vehicle to visit clients and liaise with different agencies.

Requirements:

- Must be a registered HCPC Occupational Therapist.
- Possess a valid driving licence and have access to a vehicle for work-related travel.
