What you will do:

  • Work across clusters to support Discharge to Assess referrals, managing approximately 4 cases per week.

  • Handle a mix of complex and standard Occupational Therapy cases, applying your specialist knowledge and skills effectively.

  • Demonstrate expertise in specialist seating, manual handling equipment, and in implementing simple and complex adaptations.

  • Conduct holistic assessments and develop support plans, providing reviews for individuals with a range of health conditions and their carers.

  • Utilise a sound knowledge of the legal social care framework and relevant housing legislation.

  • Maintain HCPC registration and use your ability to drive to attend various locations for assessments and appointments.
