What you will do:

Provide support under the supervision of qualified occupational therapists to help individuals gain independence and improve daily living skills.

Assist in the assessment and implementation of reablement plans, contributing to positive outcomes for clients.

Work collaboratively with a variety of stakeholders at all levels across several organisations, ensuring the highest quality of intervention.

Ensure that any specialist equipment provided is used effectively to improve independence or ease of care.

Deliver accurate verbal and written reports while maintaining IT records and correspondence to agreed standards and time frames.

Uphold confidentiality and data protection guidelines, crucial within council operations.
